二、申請(qǐng)人的資格要求
2. Qualification Requirements for Bidder

(a)滿足《中華人民共和國(guó)政府采購(gòu)法》第二十二條規(guī)定；

(a)Meet the provisions of Article 22 of the "Government Procurement Law of the People's Republic of China";

(b)落實(shí)政府采購(gòu)政策需滿足的資格要求：無

(c)本項(xiàng)目的特定資格要求：（1）符合《中華人民共和國(guó)政府采購(gòu)法》第二十二條的規(guī)定。
（2）未被“信用中國(guó)”（www.creditchina.gov.cn）、中國(guó)政府采購(gòu)網(wǎng)（www.ccgp.gov.cn）列入失信被執(zhí)行人、重大稅收違法案件當(dāng)事人名單、政府采購(gòu)嚴(yán)重違法失信行為記錄名單。
（3）單位負(fù)責(zé)人為同一人或者存在直接控股、管理關(guān)系的不同供應(yīng)商，不得參加同一合同項(xiàng)下的政府采購(gòu)活動(dòng)；為采購(gòu)項(xiàng)目提供整體設(shè)計(jì)、規(guī)范編制或者項(xiàng)目管理、監(jiān)理、檢測(cè)等服務(wù)的供應(yīng)商，不得再參加該采購(gòu)項(xiàng)目的其他采購(gòu)活動(dòng)。
（4）法人依法設(shè)立并領(lǐng)取營(yíng)業(yè)執(zhí)照的分支機(jī)構(gòu)參加政府采購(gòu)活動(dòng)應(yīng)當(dāng)經(jīng)由其法人授權(quán)。法人與其分支機(jī)構(gòu)或者屬于同一法人的不同分支機(jī)構(gòu)不得參加同一合同項(xiàng)下的政府采購(gòu)活動(dòng)。
（5）其他特定資格要求：
如果投標(biāo)人是投標(biāo)產(chǎn)品制造廠家，應(yīng)提供《醫(yī)療器械生產(chǎn)許可證》；如果投標(biāo)人是投標(biāo)產(chǎn)品經(jīng)營(yíng)企業(yè)，應(yīng)提供《醫(yī)療器械經(jīng)營(yíng)許可證》或《第二類醫(yī)療器械經(jīng)營(yíng)備案憑證》（若產(chǎn)品為二類或三類醫(yī)療器械）（僅適用于包件1、2、3、4）。        

(c)Specific qualification requirements for this program: (iii) If the unit leader is the same person or if there is a direct controlling or management relationship between different suppliers, they shall not participate in government procurement activities under the same contract; Suppliers who provide overall design, specification preparation, project management, supervision, testing and other services for procurement projects shall not participate in other procurement activities of the procurement project.(iv) Branches established by legal persons and obtaining business licenses in accordance with the law shall be authorized by their legal persons to participate in government procurement activities. A legal entity and its branches, or different branches belonging to the same legal entity, shall not participate in government procurement activities under the same contract.(v)provide a Medical Device Production License(manufacturer) / medical device business license or 2 class medical device business (operating enterprise)(package1.2.3.4）        

(i)符合《中華人民共和國(guó)政府采購(gòu)法》第二十二條的規(guī)定；

(i)Comply with the provisions of Article 22 of the"Government Procurement Law of the People's Republic of China";

(ii)未被“信用中國(guó)”（www.creditchina.gov.cn）、中國(guó)政府采購(gòu)網(wǎng)（www.ccgp.gov.cn）列入失信被執(zhí)行人、重大稅收違法案件當(dāng)事人名單、政府采購(gòu)嚴(yán)重違法失信行為記錄名單；

(ii)Not included in the list of dishonest persons subject to enforcement, the list of parties to major tax violation cases, or the list of records of serious illegal and dishonestacts in government procurement by "Credit China" (www.creditchina.gov.cn), China Government Procurement Network (www.ccgp.gov.cn) ;
